What is Chronic Pain?

Chronic pain is long standing pain that persists beyond the usual recovery period or occurs along with a chronic health condition, such as arthritis. Chronic pain can manifest in various forms, affecting various parts of the body. Some common types include;

Neuropathic Pain

Neuropathic pain is often difficult to manage because there is not always an obvious cause, it can be triggered by such a wide range of conditions, and it is difficult to describe symptoms in a way that is meaningful to somebody else (a sensation or individual response to stimulus)

Neuropathic pain results from damage or dysfunction in the nervous system, and presents in many different ways, examples include:

  • Spontaneous pain with no apparent cause burning, stabbing, or electric shock-like sensations
  • Allodynia, where normally painless stimuli, such as a light touch or pressure, cause pain
  • Dysesthesia, which refers to any unexpected sensations that are unpleasant or painful
  • Hyperalgesia, where a painful stimulus, like heat, causes an exaggerated pain response

Examples of conditions associated with neuropathic pain include :

  • Migraine
  • HIV
  • Diabetic neuropathy
  • Facial nerve conditions such as trigeminal neuralgia
  • Postherpetic neuralgia
  • Complex Regional pain syndrome
  • Cancer related pain
  • Central nervous system disorders such a multiple sclerosis
  • Spinal nerve compression or inflammation such as herniated disc
  • Phantom limb pain occurring after amputation
  • Nerve damage following surgery, trauma, radiotherapy
  • Some chemotherapy drugs trigger neuropathic pain

Musculoskeletal Pain

Musculoskeletal (MSK) conditions affect many people and can affect your joints, bones and muscles and sometimes associated tissues such as your nerves. They can range from minor injuries to long-term conditions. Symptoms can include pain, stiffness, limited movement, and disability which affect quality of life and independence.

Examples of conditions which cause MSK pain include:

  • Fibromyalgia
  • Tendinitis
  • Spondylolysis
  • Chronic gout
  • Chronic bursitis
  • Carpal Tunnel Syndrome
  • Cancer related pain
  • Ehlers-Danlos syndrome

Visceral Pain

Visceral pain, sometimes described as deep-seated pain is a type of pain which originates from internal organs such as the stomach, intestines, bladder or reproductive organs. This type of pain is often described as crampy, gnawing, twisting or a feeling of intense and painful, deep pressure. Pain receptors in visceral organs are not as dense or evenly distributed as they are in others area making the origin of the pain hard to pinpoint and challenging to manage.

Visceral pain can be related to:

  • Endometriosis
  • Crohn’s disease
  • Colitis
  • Ulcerative Colitis
  • Uterine fibroids Interstitial cystitis
  • Primary dysmenorrhea
  • Irritable bowel syndrome
  • Cancer related pain
  • Chronic pelvic pain syndrome/ prostatitis
  • Chronic orchialgia

Radicular pain

Radicular pain refers to symptoms such as burning or shooting pains which radiate along a nerve path due to irritation or compression. Radicular pain is a symptom of an underlying radiculopathy.

  • Cervical Radiculopathy occurs when a nerve root in the neck is compressed which can cause pain in the arms and hands
  • Thoracic Radiculopathy involves the nerve root in the upper back which can cause pain the chest and torso
  • Lumbar Radiculopathy is affecting the nerve root in the lower back which can cause radiating down the leg and into the feet

Radiculopathy can arise from

  • Herniated discs
  • Spondylolisthesis
  • Bone spurs
  • Traumatic injury
  • Degenerative disc disease
  • Degenerative spinal change

Psychogenic Pain

Associated with psychological factors like stress, anxiety, or depression. It can exacerbate existing pain conditions or manifest as unexplained pain.

Central Pain Syndrome

Caused by dysfunction or damage to the central nervous system, such as the brain or spinal cord. Conditions like multiple sclerosis and strokes can lead to central pain.

Mixed Pain Syndromes

Some individuals may experience a combination of different types of chronic pain, making diagnosis and treatment more complex.
